Brain Training Enrollment Agreement

By purchasing this package, you are enrolling in Peak Potential Personal Development’s year-long neurofeedback brain training program. This program is designed to support the development of calm, focus, and confidence from the inside out.

Payment Terms

Clients agree to pay $495 per month for a duration of 12 months. Payments will be automatically processed each month starting from the date of this agreement. Training for a year gives your brain enough time to really create new neural pathways and lasting change. Your brain will actually be learning during your training sessions and all new skills need time to develop.

Cancellation and Unit Return Policy

Should you choose to discontinue treatment, you must provide at least 15 days' notice before your next payment is due. Additionally, you are required to return the neurofeedback unit in working condition within 15 days of cancellation. Failure to return the unit may result in additional charges. Because of my financial commitment to purchase credits, accounts, and the equipment charges I incur to make the training accessible for you I am unable to offer refunds for payments that have already been charged.

Program Inclusions

During your time in the program, Peak Potential Personal Development will provide the following:

• A neurofeedback unit, which is on loan to you for the duration of your training + tech support

• Up to twelve (12) sessions of neurofeedback per month

• Initial training and ongoing support and monitoring of equipment and programs

• Monthly one-on-one 30-minute support sessions to support progress
